We built our intake process to remove barriers, not create them. Be honest with us. Be real with us. We read every word — not to judge your past, but to understand where you are and prepare for your success.

What Happens

When You Apply

Step 1 — Complete the Application

Fill it out completely and honestly. If something doesn't apply to you, write N/A. Blanks are okay — just be as complete and honest as you can. Every section exists for a reason — to help us serve you better.

Step 2 — We Review Your Application

We take every application seriously. Our team reviews every application carefully to understand who you are, what you need, and whether Pivot Pad is the right fit for where you are right now.

Step 3 — Your Story Starts Here

If it's a good fit, we'll get you placed and get you started. The life you're building begins the moment you walk through the door.

Who You Are

Basic personal and contact information.

Your Supervision Status

If you're coming from IDOC, on parole or probation, or self-referring, we want to know your situation so we can coordinate properly.

Your Background

We don't disqualify people for their past. We ask because honesty is the foundation everything else is built on.

Health, Employment & Support

Where you are practically — ID documents, employment, medical needs, and financial plans.

Addiction History

Honesty here is not a disqualifier. It helps us serve you better. We ask because we care about your recovery, not to judge your past.

Faith & Preferences

Pivot Pad is a Christ-centered program. Faith is not required to participate, but respect for its values is.
