You Don’t Need to Have It All Together to Take the First Step. You Just Have to Take It.

Your Application
"Have I not commanded you? Be strong and courageous. Do not be afraid; do not be discouraged." — Joshua 1:9
Taking this step takes courage. We know that. Whether you're stepping out of treatment, preparing for release, or simply ready to stop living the way you've been living — this is where it starts.
We built our intake process to remove barriers, not create them. Be honest with us. Be real with us. We read every word — not to judge your past, but to understand where you are and prepare for your success.
What the
Application Covers
We ask thorough questions because we care about getting it right — for you and for the community you'll be joining. Here's what you'll share with us:
Who You Are
Basic personal and contact information.
Your Supervision Status
If you're coming from IDOC, on parole or probation, or self-referring, we want to know your situation so we can coordinate properly.
Your Background
We don't disqualify people for their past. We ask because honesty is the foundation everything else is built on.
Health, Employment & Support
Where you are practically — ID documents, employment, medical needs, and financial plans.
Addiction History
Honesty here is not a disqualifier. It helps us serve you better. We ask because we care about your recovery, not to judge your past.
Faith & Preferences
Pivot Pad is a Christ-centered program. Faith is not required to participate, but respect for its values is.
Your Personal Statement
This is your opportunity to speak for yourself. Tell us who you are, what you're committed to, and why Pivot Pad is the right next step for you.
We're Ready
When You Are
This isn't just paperwork. It's the beginning of something real. Take your time, be honest, and know that someone on the other side of this form is genuinely rooting for you.
"Perfect doesn't get done, but done is perfect."
Stop waiting until you feel ready. Fill it out. Send it in. We'll take it from there.
"For I know the plans I have for you, declares the Lord, plans to prosper you and not to harm you, plans to give you hope and a future." — Jeremiah 29:11
